DIS 499/599: Module 4: Topic 2: Web Activity
Week 10: 3/25 - 3/31
Objectives:
* To identify local, state and national service agencies and organizations that provide support and services for individuals with developmental disabilities and their families
To complete this assignment successfully, you should:
1. Your role as resource specialist for a local agency is to attend a Family Support Interagency Council meeting next week. You have been asked to present information that fellow members can use to plan services for the Martinez family (see Case Study presented in the Online Lesson).
2. Use the Internet, phone book, and/or library to find one local, one state and one national agency (public/government run) that provides services to individuals with developmental disabilities and their families.
For each of the agencies provide:
a. the full name of organization
b. the full address and phone number for each agency
c. a contact person or specific office within the agency
(Think about all of the articles you have read to determine what type of resource information and services would help the Martinez family. How could the agencies you chose help this family?)
d. include a brief statement of why this agency would be a good place to refer the Martinez family
3. You have been asked to distribute this information to your colleagues prior to the meeting by emailing your report no later than midnight on Sunday, March 31st to Becky.Raabe@nau.edu.
4. This assignment is worth 10 points.
