Application Information and Materials

Applications must be complete, and portions that must be sent to the program office must be postmarked no later than January 5, 2007. 

The on-line application can be found at http://www.applyweb.com/apply/northazg/.  Please note, you will still need to print portions of the application after completion, download the Letters of Recommendation Forms (and have them completed), print your GRE score sheet, and mail those items to the Physical Therapy Program as a complete packet.
