    1. Documentation of a completed Associate degree in an Allied Health Field (unofficial transcripts are acceptable)
      AND
    2. Copy of current state or national license (in the same allied health field as the AAS).

  1. Proof of current enrollment in an Associate degree in an Allied Health field (unofficial transcripts are acceptable)

  2. Proof of waitlist status in an Associate degree program in an Allied Health field from a regionally accredited college or university. A copy of an official college document or a copy of an email from the college or advisor stating waitlist status is acceptable.
