be your own work.
The hardest cases will fall between those who are barely conscious and the
pleasantly senile. For those patients it may be difficult to ascertain or meaningfully
discuss their interests in living from their own limited perspective. "Objective"
quality of life assessments are inevitably crude and subject to dispute. Disagreement
and controversy may be unavoidable. In some cases, it will be impossible to
reach a clear decision on what outcome would best serve the patent's existing
interests. Moreover, extensive debate may be required to determine what level
of awareness and benefit is sufficient to give incompetent patients a significant
interest in continued life, notwithstanding the patients' former competent preferences
against treatment and the burdens imposed on family and society. (241)
