DENTAL HYGIENE APPLICATION INSTRUCTIONS

INSTRUCTIONS FOR COMPLETING THE
DENTAL HYGIENE APPLICATION

Applications to the Dental Hygiene Program at NAU will be accepted beginning December, 2009. The deadline for accepting application is February 1st, 2010 (applications MUST BE POSTMARKED BY FEBRUARY 1st, 2010).

DEADLINE FOR RECEIVING APPLICATIONS:

February 1, for admittance in the Fall. Materials postmarked after this deadline will not be accepted. Please note that you are responsible for distributing AND collecting the academic record request forms. These materials must accompany your application in their signed and sealed envelopes, or the application will be considered incomplete and returned to the applicant. Be sure to allow adequate time to distribute these items in order for them to be returned to you for inclusion with your application.

COLLEGE TRANSCRIPTS:

The first step to submitting a successful application is to request your college transcripts. You must submit one (1) official transcript of your academic record from each college or university you have attended. If you have any questions about transcripts please contact an advisor. Every applicant must send all new transcripts, as the department does not retain these from year to year. The transcript can be considered official only if it is unopened in the institution's envelope and stamped with the seal of the college or university.

Follow these instructions very carefully:

1. Complete an academic-transcript request form for every college or university attended.
2. Send the completed form to the registrar of each college or university you have attended. Be aware that most universities will charge you for this service, so be sure to contact them concerning the necessary fee that needs to be included. Failure to do so may cause you to miss the application deadline.
3. When the completed academic-record form and transcripts are returned to you in the sealed envelope by the registrar, place these UNOPENED envelopes in a large envelope along with your other application materials.

   IMPORTANT:

   DO NOT OPEN ANY TRANSCRIPT ENVELOPES YOU RECEIVE FROM THE REGISTRAR. IF THE SEAL IS BROKEN ON THE ENVELOPE, YOUR ENTIRE APPLICATION WILL BE RETURNED TO YOU.

4. If you are enrolled in college during the most recent fall semester, the transcripts that you request MUST contain your fall semester final grades for your application to be considered, so be sure to request transcripts from your institution immediately upon completion of the fall semester. If you are attending a college that uses the quarter system send a transcript that includes your current enrollment.

HOW TO COMPLETE THE DENTAL HYGIENE APPLICATION:

Read these instructions carefully before completing the Dental Hygiene application. Failure to comlplete application exactly as stated will result in deduction of points or denial of application.

COLLEGE CATALOGS (IMPORTANT FOR TRANSFER STUDENTS):

In order to evaluate the college transcripts that you will submit for application into our program, it will be necessary for us to have a catalog that describes your prerequisite science courses from each of the colleges or universities that you have attended. We have all of the catalogs for the community colleges and the universities located in Arizona. However, we do not maintain college catalogs for out-of-state universities or colleges.

Therefore, in order to ensure proper evaluation of courses you have taken outside the state of Arizona, please send to this department a copy of a catalog for each institution you have attended. Photocopied pages of the catalog with the course descriptions are acceptable.

This information is mandatory for consideration of your application to the Dental Hygiene Program. To facilitate the application process, please forward this information with your application by the deadline. If all of your college coursework has been in Arizona, you may disregard this request.

NOTICE OF RECEIVED APPLICATION:

Complete the top portion of this notice. Include a self-addressed stamp enveloped with this notice when returning your application. It will be sent to you to verify receipt of your application or identify any additional credentials needed.

ADMISSION TO NORTHERN ARIZONA UNIVERSITY:

If you are accepted into the Dental Hygiene program and are not currently enrolled at NAU, you must also be admitted to NAU. This is a separate application procedure and you need to contact Undergraduate Admissions, PO Box 4084, Flagstaff, AZ 86011-4084 for information and an undergraduate application. It is not necessary to be admitted to NAU prior to applying to the Dental Hygiene program. However, admission to NAU is required in order to begin the Dental Hygiene program. You will need to send OFFICIAL transcripts to the NAU Registrar. The registrar will not accept photocopies from the NAU Dental Hygiene Department.

NOTIFICATION OF ACCEPTANCE:

Notification of acceptance to the Dental Hygiene program will be sent to applicants in April. Should you be accepted, you must forward, within two weeks from the date of our acceptance letter, a non-refundable deposit in the amount of $500. The deposit will be applied toward the cost of your dental hygiene instruments for the first year of the program.

APPLICATION PACKET SHOULD CONTAIN THE FOLLOWING PRINTED FORMS:

1. Official Transcripts from every University/College attended. (If you have questions about submitting transcripts from Universities/Colleges that would not pertain to the DH Program please contact an advisor.)
2. Acknowledgement Notice (click on link)
3. Application Checklist for application requirements (click on link).
4. Paper application. Read the online instructions found here. Then click here to start filling out the online application form. The completed form will be emailed to you. Print it and include it in your application packet. Alternatively, once completed, you can authenticate and print your form from this page.
