HP570 : The Class : Observation : Observe : Portfolio

Assignment: Observe the Eight Components of CSH Program

Part 1 of the Capstone Paper (Due by Aug. 6)
  1. You are encouraged to work with a partner if there is a colleague from your school in the course. If necessary, you may work alone.

  2. Select a school of your choice where you will conduct the interview.  If you are currently working in a school setting plan to evaluate that school so that the capstone project is more relevant, beneficial, and meaningful to you. If the principal or assistant principal is not available to interview, think of alternative ways that you could obtain the information required to conduct the assessment (ie: accessing school records, program summaries, etc. through the school office)
  3. E-mail the instructor by July 11 and receive approval for the school you plan to evaluate under the subject heading of "Capstone Overview." Provide a brief overview of how you plan to conduct your evaluation (School Health Index and/or Principal/teacher interviews, etc.

  4. Choose from 1 of the following 2 options:

    Option 1:
    A. Develop a survey of 1-2 questions on each of the 8 CSHP components that you will ask in an interview with a principal or other designated individual (s) at a school. Use the essentials of a quality CSHP as discussed in Health is Academic and the related web sites as you develop the survey for the interview .


    Focus on:
    • The extent to which the school is focusing on reducing the prevalence of the six key risk behaviors.
    • The extent to which the essentials of each of the 8 CSH components are being followed by the school.
    • The extent to which there is coordination among the 8 CSH components.

    B. Contact the principal (or other administrative person, teachers with knowledge of information sought (year round schools) and establish a time for the interview. Let the principal know that you are taking a course on School Health Programs and your interview is part of the capstone experience for the course. Bring the School Health Index and other relevant course/web materials to describe the model.

    C. Conduct the interview

    D. Record your responses to the questions

    E. Record the school where you conducted the interview including full address, the number of students in the school, the name of the principal , teachers etc. you interviewed, and the date of the interview. 


    Option 2:

    A. Meet with the appropriate school staff and complete one module (component of CSHP) of the School Health Index. (If your school has already made some progress towards CSHP, this would be the recommended approach.)
    Physical Activity, Healthy Eating, and a Tobacco Free Lifestyle can be assessed using the School Health Index.


    B. Based on the results of the questionnaire, complete the Planning Questions and Recommendations Chart (included under each module of the school health index).


    C. Write a summary of the process that includes:Name and title of person(s) providing the information needed to complete the assessment.
    How long it took to complete the assessment. The reaction of the participant(s) Your reactions and observations.
    Was it a "user-friendly" assessment tool? Would you use it again to assess other CSHP components in the areas of Physical Activity, Healthy Eating, and a Tobacco Free Lifestyle? Why or why not? Other comments?
